You must include an e-mail address so I can send you your passwords. You can include a credit card number and expiration date, but you must include the name on the card and address to which the card is billed, in order for me to process a credit card through the mail.

Remember they don't call it SNAIL MAIL for nothing. You must allow a week or so for the mail to be delivered.
